    This is an easy mistake to make, though a mighty embarassing one. The (large) website I work for had this very same problem recently. If your error pages don’t return the correct HTTP response code, Google’s search spiders will repeatedly hit them trying to follow links it shouldn’t be (members-only pages, broken links from external sites, any links during a site outage, etc). Thus, the top page quickly becomes an error page. Whoops.
